Cosmetic Surgery Cancellation Policy

Fraxel:

In order to reserve a date for you procedure a 1/3 deposit is required. Full payment is due no later than 2 weeks prior to your procedure date.  We cannot accept a personal check greater than $300 on the day of your procedure.
Due to the high cost of materials and our inability to return them to the providing companies, full refunds will be made only within 48 hours of scheduling. For up to 6 months after the initial 48 hours, a refund will be granted minus the cost of materials and 20% service charge.  There will be no refunds after 6 months.
In the case of Fraxel Series, once the series has begun, there will be no refunds but credit may be applied for up to 6 months. After 6 months all credit will be forfeited.

Botox, Filler Injectables and Chemical Peel Cancellations:

A deposit will be returned only within 48 hours of being made. After this time no refund will be granted.  Deposits may be applied as credit to other services/products for up to 6 months. After 6 month all credit will be forfeited.  We cannot accept a personal check greater than $300 on the day of your procedure.

Surgical Cancellations/Change Date:

In order to reserve a date/schedule a surgical procedure a deposit of no less than 1/3 of the total procedure cost must be made. This deposit allows us to reserve Dr. Karen’s time, the OR, purchase necessary supplies, and arrange anesthesia if necessary on the date you have selected with the Facial Plastics Coordinator.  Full payment must be provided at your pre-op appointment, approximately 2 weeks prior to your surgery. Cancellations after 48 hours of depositing for surgery, will result in a 20% (total cost of surgery + anesthesia) penalty + cost of supplies purchased by Exclusively Faces for your procedure ie. Implants and Fillers (Restylane, Juvederm, Radiesse, etc.)  Should you need to reschedule your date less than 2 weeks before surgery, a 3% administrative fee will be required at that time.  If surgery is cancelled less than 2 weeks prior then 50% of the total cost is forfeited.  If surgery is cancelled with less than 72 hours notice then the total cost is forfeited.

In the event surgery is cancelled emergently because of extenuating circumstances ie. severe illness, accident etc. every effort will be made to reschedule and accommodate the patient.

In the event a deposit is made and surgery is not scheduled within 6 months the deposit will be forfeited.

Deposit/Balance Due Date:

Deposit will be due at time of scheduling your surgery date.  The balance will be due at pre-op appointment. Surgery must be paid in full at this time. 

Consultation Appointment Policy:

Exclusively Faces Cosmetic Surgery and Medispa offer complimentary consultation. In order to reserve an appointment, a credit card guarantee is required.   A $150 penalty fee will be charged to the card ONLY in the event that appointment is MISSED without rescheduling or cancelling 1 full day prior to the appointment.   This policy helps us to be available for patients who want to be seen in a timely fashion.

Medispa Cancellation Policies

Facials/Massages:

There will be no refunds offered for Facials or Massage under any circumstances.  In the case of purchasing any spa package, partial credit will be given for 6 months only.   After 6 months all credit will be forfeited.

Obagi: No Returns

In the rare event of a significant allergic reaction seen and confirmed by our trained staff, Exclusively Faces will appeal to the manufacturer on your behalf. If and when Obagi replaces product to our office, credit will be granted. No Cash refunds will be granted.

Latisse:  No Returns/Refunds

Jane Iredale:  No Returns/Refunds
