Are Eyelid Surgeries Truly Necessary or Purely Cosmetic?

Blepharoplasties, known commonly as eyelid surgeries, is a more common procedure than you might think.

Usually done for cosmetic reasons — typically droopiness in the upper lids or bagginess in the lower lids — eyelid surgeries can actually also be done for medical reasons.

What Exactly Are Blepharoplasties and When Are They Necessary?

Blepharoplasties are a surgery that remove excess skin, and sometimes also remove excess muscle and fat.

For example, if the patient is diagnosed with dermatochalasis (excess eyelid skin) or blepharoptosis (downward displacement of the eyelid margin), they might decide to get a blepharoplasty.

In the case where neither of these diagnoses is hampering the patient’s lifestyle, the surgery would be cosmetic.

However, based on a patient’s subjective reports, either of these conditions might get in the way of the patient’s daily living.

Primarily, blepharoplasties may be necessary if the patient’s vision is obscured.

Does the patient have trouble reading? Can they drive without issue? Does the obscured vision perhaps cause the patient headaches throughout the day?

All of these matter when it comes to determining if surgery is appropriate.

Surgeons Can Take Objective Measures With Eyelid Surgeries Too

While the most important factor is the patient’s reportage of difficulties, there are objective measurements doctors can make to help determine if surgery is appropriate.

For example, the distance between the edge of the eyelid and the center of the pupil is called the margin reflex distance.

If this distance less than 2mm, this could be an indication for blepharoplasty.

A chin-up head tilt can be a strong indication that a patient is trying to ‘see past’ their eyelid.

This can also be a sign that eyelid surgery might be necessary.

Similarly, another indication is if the patient is experiencing superior visual field loss of at least 12 degrees.
